Software glitch? Whats the true story?

Hi all geeks and others..I am particularly interested in this topic.  A payroll system is not one that should be taken lightly especially for a large entity like the government.  So I can understand the frustration of those affected.  But what is the real story?  Is it really a glitch?  Does anyone know which system is at fault.  It seems like SStar is saying the new system made the error while others say its the old system.  In any case, the ideal scenario for any deployment of software is to TEST, TEST and TEST again.  I wonder if enough testing was done to try every possible scenario...Someone is at fault and some did not do their work properly.  Anyone can share some more insights into this?  Or perhaps a geek with more knowledge can put a technical spin to this discussion. Thank you.

This sounds like and I/O error (Idiot Operator error).
